- Enter your Lab Account Number (your lab account number can be located on the top of your chain of custody form)
- Enter your email address which will be your new username

Enter a new password with the following requirements:
- At least 8 characters long
- At least one upper case letter
- At least one lower case letter
- At least one digit
- At least one of these special characters @#$%^&+=
